Appointment Information

I may ask you to obtain permission from your medical provider prior to your first session. The link to that FORM IS HERE.

At your first appointment we will review your health history and your goals together – please allow an additional 15 – 20 minutes.

Before you come for your first appointment I will email you a link to complete an online health history. This is a secure HIPAA compliant form that is not accessible to anyone except me. Please complete and submit your form before your first visit. That will allow more time for your first massage!


At each appointment we will discuss the current state of your health and what therapy would most benefit you at that time. Please feel free to discuss any aspect of your health and any events in your life which may be impacting your health. The confidentiality of the client-therapist relationship is held sacred.


Please arrive on time for your appointment and be mindful that there may be a client just before and after your scheduled time.

Cancellation Policy

I understand that unanticipated events happen occasionally in everyone’s life and I will do my best to work with you if you need to reschedule your appointment. In order to be fair to all clients, the following policies are honored:


24 hour advance notice is required if you will not be able to make your appointment. This allows someone else the opportunity to schedule an appointment. If you are unable to give 24 hours advance notice you will be charged the full amount of your appointment. This amount must be paid prior to your next scheduled appointment.


No Shows

If you miss your appointment without notice, you will be charged for your missed appointment.


Late Arrival

If you arrive late, your session may have to be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, we’ll determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session. Out of respect and consideration to your therapist and other clients, please plan accordingly and be on time.
